Am I supposed to submit annual income statement for my income from a single employer in Turkey?

A taxpayer who has business income on real taxation system in 2018 also works in a company as a 4A employee. Is this person also supposed to include their income from wages on the income statement they will submit for their business earnings?

If the income is earned from a single employer, it is not written on the annual income statement notwithstanding the amount.
